Getting Error when importing contacts from Streak

I saved my contacts from Streak to a .csv file and I keep getting an error message saying that Teamzy was not able to upload them. Can you please advise?

  Hi Dianne,

  Make sure you have First Name and Last Name separated into separate columns. This is the most common issue.  Also, make sure each column has a label, e.g. First Name, email, phone, etc..
